I accept that Red Zone Football Academy LLC., is only providing me and/or my minor child an opportunity to utilize our programming for recreational use. In consideration for the privilege to use this programming, I agree to assume all risks for myself and/or my minor child and release and hold harmless Red Zone Football Academy, its staff, agents, owners, officers, manufacturers of equipment and materials, property owners, league directors, coaches, team managers, officials, sponsors and any other having an interest in the program from all liability, negligence, causes of action, claims, demands and damages of every kind which may arise out of my participation and/or my minor child’s participation in any and all activities with Red Zone Football Academy.. I will familiarize myself and/or my minor child (if applicable) with the rules of the game and/or the program and facility policies and will, to the best of my ability, have him/her play under control and avoid injury to self and other persons while using the facility. I have carefully read, understood and accepted all the policies of Red Zone Football Academy LLC. All adult participants must sign this waiver form to actively participate in any programs offered by Red Zone Football Academy LLC.,. For minor participants who are under age 18, a parent or guardian must complete and sign this waiver form on their behalf. By signing you indicate your/their willingness to accept responsibility for the conduct and sportsmanship while participating in Red Zone Football Academy LLC. programming. THE TERM OF THIS WAIVER IS INDEFINITE, UNLESS WITHDRAWN BY THE ADULT PARTICIPANT SIGNING OR THE PARENT OR GUARDIAN OF A MINOR PARTICIPANT UNDER 18. Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact as well as through perspiration and respiration. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Red Zone Football Academy LLC., has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that you or anyone you come in contact with after leaving the facility will not become infected with COVID-19. Further, attending Red Zone Football Academy LLC. could increase your risk and those you come in contact with risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that myself and anyone I come in contact with may be exposed to or infected by COVID-19 by attending the Red Zone Football Academy LLC. and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Red Zone Football Academy LLC. may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Red Zone Football Academy LLC. employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection my attendance at the Red Zone Football Academy LLC. or participation in Red Zone Football Academy LLC. programming (“Claims”). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless the Red Zone Football Academy LLC., its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Red Zone Football Academy LLC., its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Red Zone Football Academy LLC. program.
